I just can't get SA on Flatline. I always get Hitman, because I used too much violence. I don't know how I used violence, though; I only kill the targets.

I'm pretty sure it is so I'll continue, how did you kill the 3 targets before you have to go to inject the agent in his cell? I used the explosives on the roof lighting fixture (making the death appear accidental) on 1 of them, for the other 2 I used lethal injection and/or fibrewire, the bodies of these 2 remained undiscovered for the duration of the job. None of the NPCs were fully alarmed at all during the mission (makes getting to and from the agents cell a bit tricky).
This was enough to get me a Silent Assassin rating.
I haven't experienced it myself (I prefer lethal injection & fibrewire) but I've heard that on missions where its not necessary, gunshots (even with a silencer and where no one discovers the body) can lower your rating. Perhaps this was your problem?

No, here's what I do:

-Get patient's robes.
-Push the guy in pink into the jacuzzi.
-Drop the weights on the guy in the gym (wearing green)
-Strangle the guy wearing blue in his room.
-Get therapist's outfit.
-Go down to basement. When the therapist goes upstairs and the orderly (not the one in the room, the one with the coffee cup) walks past the laundry basket, sedate him and take clothes/ Cell Key. Hide body in basket.
-Go to agent Smith, talk to him, sedate him without being seen.
-Go outside, wait till he reaches the morgue, wake him when the orderly leaves, and exit.

All without being seen, except the guy in the gym is found. But I thought it doesn't matter when people who died by accident are found. I get one bar of violence on the missions complete screen.

I attempt to kill my target with stealth, but if anyone sees me when they shouldn't, I won't hesitate to shoot them down for the sake of the mission.

i try to do it it silently then i get caught so i kill every1.... but i usually get silent assassin...

I like to double hit levels , go in waste every thing and colect my weponds ,and locate any thing i need to retrive or that may help me then go back un armed and watch my maps, study the movements of my targets, gards and other trafic so i can slip about and take out my tyarget(s) as quiet as a mouses ghost
