I'm assuming you mean online, because "lag" offline simply refers to a slowdown in frame-rate from the engine being worked too hard.

In regard to the former, the online only lags if you have a poor connection or are far away from the host. Usually, the game will match you with a host who's nearby to alleviate this problem, but there's always the chance you will get a bit of lag in an online game from time to time. GTAIV is no different than your average online game in this category.

The frame-rate will dip in the single player if you've got a ton going on on the screen at once, but it's hardly noticeable. You can count down on this by installing the game to your hard drive before you play it. In my experience, this keeps the game from "freezing" (which it is wont to do from time to time) and improve the frame-rate during shootouts.
