Now that I finally beat it, I realized it was quite easy.

Just transplant the liver without worrying about the Stigma so that vitals can rise, then destroy Onyx, which always cut the one with 4dots, then sew him up and destroy Cheir.

I think the worst operation thus far is either the one with Onyx and Cheir at the sametime, or the one where the farking DOG gets shot with a shotgun.
