Yeah play through all the missions in Portland and you will cross over to Staunton Island eventually.

ive had the game 5mins and im already there but youd best focous on Slavatores misions he gats u there u take him over. At the begining of the mission hell threten you with a hand gun askin wat the f*^k youve been doing and stuff then u take himn on a joyride and then over the bridge with lvl3 heat and once your over go to the new safe house

On that last Salvatore mission, the jump must be done right, otherwise you'll fall into H2O and die.
