Whoa! Calm down and eat some Rat Meat.
You first have to go to Crucible and look for a store called "Cutter's Weapons", from there, she will give you all the information you need, as well as make you weapons and armor according to how much Madness Ore you have. She can make equivalent weapons and armor with magical enchantments if you give her a Matrix along with the ore.j
Like you give her 4 Madness Ore for a sword, you get the sword.
Or you can give her 4 Madness Ore and a Madness Sword Matrix and you'll get a sword with a magical enchantment (as far as I know, you can't control what effect you get)
As for finding the stuff. There's a chance that Grummites will hold the Ore as well as Matrices.
Or you can visit any Ruin/Fort/Cave that has Grummites, and most of the chests/urns will have Ore in it.
OR! In said Ruins/Forts/Caves, you sometimes see a glowing green light. Well, if you go over there, you'll find a Madness Ore deposit, you can get anywhere from 1-3 pieces from it.
Don't forget about Amber too. Just go to Bliss and look up "The Missing Pauldron" store. Now just put everything I wrote up there down here, with a few exceptions.
You can find Amber in Root Caves w/ Gnarls, or on Gnarls themselves.
Look for a glowing yellow/orange light, this'll take you to a Amber Root, and you'll get 1-3 pieces.
One last thing.
Amber is Light Armor.
Madness is Heavy Armor.
PHEW! There ya go!

In the Shivering Isles, you have to discover everything first.
But once you get past the Gatekeeper, if you are lucky, you'll find either a Golden Saint, or a Dark Seducer. Punch one of them, and you'll get the choice of paying your fine. Do this, and they'll automatically take you to their appropriate city.
Bliss, Crucible and New Sheoth are to the east.
Bliss is the northern city.
New Sheoth is in the middle.
Crucible is to the south.
But I say this lightly, because all three cities are really one big city with door connectors to each part.

I remembered I could do that AFTER I got most of the Madness and Amber armor....I went through all that too.

It depends on what you like. Madness looks like what it's name implies....a psycho face on the front of your cuirass.
Amber is really pretty (especially the Bow), it's really glittery and golden-orange colored.

I don't like how it looks either. It reminds me of the Defender Shield from the first Kingdom Hearts.
Yeah, amber's nice. I just like how the bowstring looks when the light hits it.
*sigh*

Be sure to stick with either Madness or Amber until the shopkeeper likes you enough to sell you perfect armor and finally grand armor and weapons. Do NOT go to both unless you just want regular armor. Stick with one until you can get all the grand in that type of armor and weapons and then work on the other one.

Likes you? I thought what you got was level dependant. Anyway, I don't like amber looks, it looks too much like glass. I do like madness though especially with count ciron's helm...

You get Perfect Madness/Amber armor depending on if they like you or not?!
Well, that would explain that one quest in Dementia, where you have to help those Ghosts reclaim their Fort from some Heretics. The stuff that came off of them wasn't 'Perfect'...

Well, I haven't had the time to fully experiment with this...
I know Perfect Madness/Amber armor is the best you can get.
And I remember bumping Cutter/Orc-dude's Dispositions over 70 (cause I was training my Speechcraft), and the Armor they gave me was automatically Perfect.
But in the field, I've run into different levels of Madness/Armor, I can't remember the names. But if you've noticed the Knights of Order and how their swords vary in name: i.e. Immaculate, Perfect...etc, the Armor is the same.
As a side note, the same can be said about Golden Saint's/Dark Seducers/Grummites, their Bows, Swords, Maces, Clubs and Arrows all have different levels of "perfectness". Since I've run into many different varieties at my character's level 38, I have to say that these aren't dependent on your level.
I can't say the same for Madness/Amber armor though...but I will once I play around with it.
Does that help? Verbose I know....

According to UESP wiki, it's level dependant, however that doesn't explain the ghosts that have madness weapons, but they are grand and perfect for me, being a level 51 darkelf
