I also thought the one where you had to transplant the two livers into those people was very annoying too. >.>

That one was actually kind of easy.
The one I found most difficult the first time through was 3-2...trying to put that stupid patch on while the target area pulsed and revolved. >_<

Is it just me, or are some of these operations Un-S-able? I do everything perfectly on operations 1-2, 1-5, 2-1, and 4-3 and the best i always get is an A.

I've XS'd or at least S'd everything, so no, its possible. Remember, time, vitals, and chain count a lot. Don't forget to get as many COOLs as you can as well.

I've beaten Second Opinion with all S's on easy/normal and all XS's on hard, so I should be able to get S's on easy mode. On Operation 1-2, the best i can get on easy/normal is A and the best I can get on hard is S. What's that about? Some operations in Second Opinion you could be too good to get an S/XS on, but I can't see that here.

Are you getting COOLs on the tumors? Thats the most likely problem that you're having.

placing them on the tray gets me a good and i've tried putting them everywhere on the tray and i get a good

Ah, sorry. This is going in the V 1.1 of my guide tomorrow, but I'll list it here.
To get COOLs on tumors, you have to cut it out from under the organ without pinging it with the ultrasound. (You can still select ultrasound, you just can't press A) As long as you don't screw up the next few steps, you'll get a COOL.
(Its impossible to COOL the first tumor in 1-2 though, so don't even try.)

The one where you have to treat a burn victim and a lot of other stuff that I can't quite get around yet without making at least one mistake.

I think the worst operation thus far is either the one with Onyx and Cheir at the sametime, or the one where the farking DOG gets shot with a shotgun.
